Request for IU1 Mobile Career Lab Services

Please Note:

Reservation requests for the Mobile Career Lab must be made a minimum of two weeks in advance.

A new submission form will need to be completed for additional visits (one form request per visit).

Some lessons/units will require multiple visits to your school. They have been indicated on this form with an * and is also indicated on our menu of services page found on the IU1 Career Readiness Website.

General Session Information:

Please provide THREE potential dates and the time length needed for the visit. In case of inclement weather or a scheduling conflict, date option #2 or #3 may be used.

Requested Service:
